Medication Therapy Management

At Perrone Pharmacy, we work with your health care providers and insurance company to help you on a road to better health. You may have heard of the term “Medication Therapy Management (MTM)” but didn’t know what it involved. Generally, MTM is offered to those individuals with multiple chronic conditions such as diabetes, asthma, hypertension, hyperlipidemia and congestive heart failure. The MTM service is rendered by a pharmacist and the goal is to optimize medication therapy and improve patient outcomes.

When MTM is deemed appropriate for a given patient, one of our pharmacists will collect medication as well as personal information from the patient in either a phone call or in-person consult and identify medication-related problems. Then our pharmacists determine the appropriate interventions for improvement and develop goals. We often communicate with the patient’s other health care providers to improve overall health.
